What information do I need to complete the intent to apply request?

The following information is required to complete an Intent to Apply request.

Please note that the request must be completed and submitted at one time. It cannot be saved and returned to later to complete.

This form will be sent to your School Principal or Supervisor who must approve it before further action can take place. Please note: All fields are required. You cannot submit this request until all fields are completed.


Raise this request on behalf of  (The name of the grant seeker/person completing the request.)


Supervisor Approval   (The name of the grant seeker’s principal or supervisor. This person will have to approve the project for it to move forward.)


Name of Grant  (The name of the grant program—not the name of your project)


Funding Agency  (The name of the organization sponsoring the grant competition.)


Grant Website   (The web link to the grant information, if there is one.)


Overview of the Grant  (Please provide a description of the purpose of the grant, not a description of your project. There is a space below to describe your project.)


Proposal Due Date  (The date by which the grant must be submitted to the funding agency)


Project Period (if applicable)  (The time frame for the project, e.g., 6 months, 1 year, 5 years, etc.)


Estimated Amount of Grant Award  (The amount to be requested.)


Are matching funds required?   (Matching funds are what additional resources must be allocated for the project. The bottom line is that the granting agency is not going to cover the full cost of the project. Matching funds may be in-kind or cash.  In-kind match can be materials/people involved in the project that are not paid using grant funds.  Cash match is typically just what it sounds like, additional funds allocated to the project.  You will have to have your principal/supervisor’s approval to include matching funds, whether in-kind or cash. If matching funds are required, please consult with the pre-award grants team for guidance on this. The requirements vary from program to program as to what constitutes eligible in-kind/cash match.)

Are personnel positions created?  (This references positions that will be grant-funded.  You will have to have your principal/ supervisor’s approval to create new positions. Will District funds be needed to continue the program after the grant funding period ends? (If the program is designed to continue beyond the grant-funding period, how will it be funded? If district funds will be required, then the response will be yes. When developing your project, please remember to consider how it will be sustained after grant funding ends.)

Are there any issues for the risk management, safety, or law enforcement departments?  (This question pertains to whether any of these Departments may be indirectly involved in the proposed program.  If you are unsure about this, please contact the Pre-Award team for assistance.)
